What is a Lasting Power of Attorney (LPA)?

The LPA is a legal document and a deed, which allows a person who is at least 21 years of age (‘Donor’), to voluntarily appoint one or more persons (‘Donee(s)’) to make decisions and act on his/her behalf if he/she loses mental capacity one day.


A Donee can be appointed to act in the two broad areas of personal welfare and/or property & affairs matters. The LPA also gives the authority to the Donee to sign deeds on the Donor’s behalf.

How to make a Lasting Power of Attorney (LPA)?

In most cases, you can make a lasting power of attorney (LPA) by completing an application online. However, you may need to consult a lawyer to complete LPA Form 2 if you want to:

  • appoint more than 2 Donees;
  • appoint more than 1 replacement Donee;
  • grant your Donee or Donees specific and customized powers.

Prepare your LPA application online

To prepare an LPA (Form 1) application online, you’ll need a Singpass account and the details of your Donee or Donees:

  • full name
  • NRIC or FIN number
  • local mobile number
  • email address (optional)

Your Donee or Donees will be notified via SMS, and must accept or reject their appointment by logging in to the Office of the Public Guardian Online (OPGO) with their Singpass.

Visit a certificate issuer

Once all your Donees have accepted their appointment, you’ll need to certify your LPA. This can be done by visiting a certificate issuer. And we have reliable and professional Certificate Issuers in our clinic, who are ready to help at any time.

After your LPA has been certified, the certificate issuer will submit the LPA application to OPG on your behalf via OPGO.

 
Application fees

If there are application fees involved, OPG will notify you via SMS or email for payment after receiving your LPA form. You can log in to OPGO to make payment using a credit card. 

Singapore citizens who submit their LPA Form 1 before 31 March 2026 will have their application fees waived.

LPA registration

Your LPA will only be valid after it has been registered with OPG. You’ll be notified when this happens. This usually takes about 4 weeks after OPG receives your application, including the 3-week mandatory waiting period.

There is a mandatory waiting period of 3 weeks before your LPA can be registered. OPG will register your LPA at the end of the waiting period if there are no valid objection from your Donee or Donees. You may also revoke your LPA if you change your mind.

Once registered, you and your Donee or Donees can view your LPA on OPGO.

Where to Find a Certificate Issuer?

When completing your LPA, the LPA Form will have to be witnessed and certified by an LPA certificate issuer. Any one of the following professionals in Singapore can be the certificate issuer for your LPA:
A) a medical practitioner accredited by the Public Guardian;
B) a practising lawyer; or
C) a registered psychiatrist
